Tummy Tuck (Abdominoplasty)

Pregnancy and rapid weight gain causes stretching of the abdominal skin as well as the underlying muscles. This results not only in visible stretch marks and loss skin but also in weakness of the abdominal wall. Therefore, despite losing weight many women still carry a “pooch” in their lower abdomen. Abdominoplasty, or tummy tuck addresses this problem by not only removing excess skin and fat, but also by tightening the abdominal muscles, therefore, reestablishing the integrity of the abdominal wall.

One of the most common questions that I get asked is whether a patient needs a tummy tuck or liposuction. Liposuction can only remove excess fat and it does not address redundant flab or abdominal wall weakness. If you have good skin elasticity, rare stretch marks, and minimal skin excess liposuction could be a good option for you. On the other hand if you are mainly concerned about excess flabby lower abdominal skin, want to remove your stretch marks, and have minimal fat you might be a better candidate for a tummy tuck. Despite of what I mentioned, you need to have your plastic surgeon examine your abdomen before you can make an adequate decision.

I perform this procedure mostly on an outpatient basis under general anesthesia. A very low abdominal incision is made that is usually hidden under a bikini. If needed the umbilicus or the belly button will also be rejuvenated. Patients need to have a friend or a family member assist them during the first week after surgery. Depending on the extent of the surgery most patients can return to routine work schedule after 10-14 days. The abdominal skin will be numb for a few months after surgery; therefore, you should be careful not to burn it with hot water. Abdominal area will be swollen after surgery, and it usually takes several months for this to resolve, and for you to be able to see the full benefits of this procedure.

Abdominoplasty is considered a major procedure and therefore carries potential risks and complications. Like many other surgeries there is a 2% risk of infection, which we will try to minimize by giving you antibiotics. There is a very small risk of developing blood clots in the legs. This risk is minimized by wrapping your legs in massage pumps during the surgery and by having you move around after surgery in order to increase your circulation. There is also a chance that a small part of your wound or even part of the abdominal skin might not heal. Although this is rare, if it happens, your surgeon can treat it. You should avoid smoking before and after your surgery because it will hamper healing. These scars could be very faint or very noticeable depending on how your body heals.

    I have had several patients of mine who have had a similar experience with darkness of the umbilicus. I have noted that in these patients the skin color of the deep part of the umbilicus is much darker than the rest of the abdominal skin. Usually this skin color is hidden with the loose surrounding skin, and become apparent only after tightening the skin. The only remedy for this is to make the umbilicus smaller, and to make it look deeper in order to hide it.

  • Q20: Tummy Tuck Scar After 10 Weeks - Hyperpigmentation Problem

    Ten weeks ago I had a full Tummy Tuck, and 4 weeks after, I had a small correction of hematoma that didn't heal and the skin was peeled off. Around this correction, hyper pigmentation started, and now it looks really ugly. Is it normal to get hyper pigmentation in this area post-Tummy Tuck, and will it fade away in time? What can I do to make it disappear?

     

    A: Hematoma worsens hyper pigmentation but it will fade

    Any amount of subcutaneous blood such as a hematoma can cause pigmentation or hyper pigmentation, especially in a person with a darker skin. This will usually fade over 9 month.

    It is critical to avoid sun bathing or tanning during this period in order to avoid permanent hyper pigmentation.

    Keeping the skin moist with oily lubricants helps the health of the skin during this period. some doctors feel that bleaching creams such as Hydroquinon are effective but in my opinion they don't make a lasting difference. It just takes time!

  • Q21: Loose and Saggy Upper Abdomen After Tummy Tuck

    I had a full Tummy Tuck 6 months ago. After the procedure, my surgeon told me that since my belly button was 'high' to begin with, he was able to leave it intact after the Tummy Tuck. My fear is that this really translates into, "I didn't remove as much skin as I should have." I now have a taut lower abdomen, but the entire area above my navel is loose and saggy. Last week, my surgeon performed laser skin tightening on my upper abdomen (his solution). I am not at all convinced that this will remedy the problem.     A: A mini-tummy tuck does not tighten the upper abdomen

    It seems that at your initial procedure you only had a mini-tummy tuck. If you did not have an incision around your umbilicus, then the upper abdomen was most likely not touched- muscle was not tightened and the excess skin was not removed.

    I don't believe that laser tightening is going to give you the same tightening of the upper abdominal skin as a full tummy tuck would have accomplished.

    Unfortunately, you might eventually have to have a full abdominoplasty and not a mini-tummy tuck Beverly Hills.

    I don't know exactly what your surgeon did, but if your abdomen did initially appear tight after surgery, and then it started to bulge gradually, then this could be because of you gaining weight. I have had patients who have not watched their diet afterward and have gained weight. Often times, the added weight accumulates in the intestinal fat or "viseral fat," which causes the abdomen to bulge out.

    First, try to lose some weight and then talk to your surgeon.
